2026 Membership Dues Form

If you wish to pay by debit or credit card, please contact the IFDA office to complete your payment. Please note that a 3% processing fee has been added to the total for all debit and credit card transactions. If you are paying with a check, please send to the IFDA office (6909 Vista Dr, West Des Moines, IA 50266) at your earliest convenience. For any questions, please contact our office at 515-270-0130 or send inquiries to:admin@iafda.org. Once the form is complete and dues are paid in full, you (and for funeral establishments, all applicable employees) are considered members of the IFDA. Membership is non-transferable.
